Greater Visibility into Care with the Support Elders Member App
The Support Elders Member app now offers members and family caregivers greater visibility into the care process, making it easier to stay informed about support as it is delivered.Designed to simplify care management, the app allows members to maintain care plans, record reminders, track and update medication schedules, and monitor important health information. Family caregivers can also stay connected to day-to-day care activities, even when they are unable to be physically present.
For members receiving support from onsite caregivers, the app provides updates on completed tasks and daily care activities, creating a clear record of the assistance being provided. Whether it is support with meals, medication, exercise, or other routine requirements, members and families can remain informed and reassured.
This enhanced visibility reflects our ongoing effort to make care more transparent, accessible, and responsive for both members and their loved ones.
